Embed a Form
How to embed an external form into QSToolbox so that your externally generated, electronic data collectors are included in your management system.

If you’ve moved away from paper forms to electronic ones, you can use this method to ensure the forms are included in your Documents register to be managed along with the rest of your quality system documents.
This method takes one step further than adding a link to the source website from within a Page.
Most form building software (e.g. Google Docs, WuFoo) allows you to embed the form on your website by providing some custom html code for you to paste into your web page. You can easily paste this same code into QSToolbox to embed the form into a Page in the Document Manager module, thus bringing the external content into your Quality Management system.
We use this same method for videos hosted on external sites.
The first step of course is to build your form! Then you need to copy the html code needed to embed your form.
Google Docs Forms
From the edit view of your form in Google Docs, choose the ‘File’ menu, then ‘Embed…’

Then select and copy the embed code for your form.

WuFoo Forms
From the forms manager view, select the ‘share’ button for the form you want to embed.

Toward the bottom of that page you will find the code for embedding your form in a webpage. Select and copy it.

Other form builders.
Look for similar option to “share” or “embed” a form in a web page.
Embed the Form in QSToolbox
Back in QSToolbox, create or edit a Page. Within the rich text field where you would normally write your page content, look for the ‘html’ button:

This will switch you into html editing mode rather than the WYSIWYG mode (What You See Is What You Get).
Paste the embed code into this window.

Don’t forget to add a version comment and then save the Page.
What you should see now is the active and functional form displayed within the Page in QSToolbox.

If users have trouble accessing the form, check you have released it in QSToolbox and also check what view permissions you have set within the external form generating software.
